Middle School Athletics Participation Forms

We know that a sports physical often feels like a quick "check the box" task and seeing a mountain of forms for a single appointment can feel a bit overwhelming. We truly appreciate the time you spend filling these out.

While it seems like a lot, these documents—including the standard New Patient Information and the OCSD Athletic Participation Packet—are required by both federal and state regulations to ensure your athlete is safe and their privacy is protected.

Having both sets fully completed and signed before you arrive allows us to skip the waiting room delays and focus entirely on what matters most: getting your student-athlete cleared and ready for the season!
