While it seems like a lot, these documents—including the standard New Patient intake and the FHSAA EL2 forms—are required by both federal and state regulations to ensure your athlete is safe and their privacy is protected.

 

Having both sets fully completed and signed before you arrive allows us to skip the waiting room delays and focus entirely on what matters most: getting your student-athlete cleared and ready for the season!
